  • Copies of social security card for ALL household members
  • Copies of identification (driver’s license or state ID card) for ALL household members over the age of 18
  • Proof of income
  • Most recent 6-8 consecutive pay stubs
  • Most recent social security award letter
  • Most recent child support print-out
  • Most recent work first award letter
